PA State Health Requirements

Physical Exams

All students in kindergarten, 6th and 11th grades are required to have a physical exam completed and the results on file with the nursing department NO LATER than May 29th, 2026. Physical exams from July 1st of 2024 until May 29th of 2026 will be accepted. Forms are available at the link below. Physician generated forms are also acceptable.


Dental Exams

All students in kindergarten, 3rd and 7th grades are required to have a dental exam completed and the results on file with the nursing department NO LATER than May 29th, 2026. Dental exams from July 1st of 2024 until May 29th of 2026 will be accepted. Forms are available at the link below. Dentist generated forms are also acceptable.


Health Screenings

All students in ALL grades are required to be screened for height, weight and vision. All students in grades K-3, 7 and 11 are required to be screened for hearing, and All students in grades 6 and 7 are required to be screened for scoliosis. We ask if your student has a physician and/or eye doctor appointment from July 1st, 2025 until May 29th, 2026, you have the required screenings completed and the results sent to the nursing department. The screening form is available below. Physician generated forms are also acceptable, if the required screenings are shown on the form.


Immunizations

The nursing department must have proof of the below immunizations on file NO LATER than September 2nd, 2026. All students entering into the designated grade must have the required listed vaccines. If your student currently has an exemption on file, then no updated immunization record is needed. If you are unable to meet this requirement by the September 2nd date, you MUST reach out to the school nurse responsible for your student IN ADVANCE of this date to discuss other options. It is the responsibility of the caretaker to provide an updated record to the school nurse.

Dose/Immunization K 7th 12th
4 doses of Dtap(one dose on or after the 4th birthday)
4 doses of polio(fourth dose on or after the 4th birthday)
2 doses of measles, mumps, rubella(MMR)
2 doses of varicella(chickenpox) or evident of immunity
3 doses of hepatitis B
1 dose of Tdap
1 dose of MCV4(meningitis)
A second dose of MCV4(meningitis)
